About this role
AdventHealth Celebration is hiring a full-time Supervisor of Care Management (Tuesday–Saturday, 8:30 a.m.–5 p.m.) at its Celebration, Florida hospital campus.
### What you'll do
- Supervise daily discharge planning and social services operations across inpatient, observation, and emergency department settings.
- Participate in multidisciplinary rounds and drive effective discharge planning and barrier resolution with the interdisciplinary team.
- Monitor care manager documentation accuracy and regulatory compliance using reports and dashboards.
- Facilitate care conferences for complex patient transitions and placements.
- Report quality and risk issues to appropriate departments and committees in a timely manner.
- Lead hiring, onboarding, evaluation, and mentoring of care management staff.
### What we're looking for
- Bachelor's degree in social work (required); Master's degree preferred.
- Minimum 2 years of social work experience and 1+ year in direct care management.
- Working knowledge of CMS Conditions of Participation, medical necessity criteria, IMM, MOON, and Advanced Directives.
- Demonstrated ability to lead teams, manage competing priorities, and navigate complex clinical decision-making.
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification preferred.
- Proficiency with PC applications and report development.
