About this role

A full-time RN Case Manager role at UChicago Medicine AdventHealth in Hinsdale, Illinois, supporting inpatient care coordination and discharge planning across a multidisciplinary team. ### What you'll do - Participate in multidisciplinary rounds to review patient status, level of care, and discharge plans, escalating care delays to leadership as needed. - Communicate with and educate patients and families on the emotional, social, and financial impacts of illness while mobilizing community resources. - Assess readmitted patients to identify contributing factors from the patient's and family's perspective. - Facilitate patient and family care conferences in coordination with the multidisciplinary team. - Document discharge planning evaluations, ongoing assessments, MDR notes, barriers to care progression, avoidable days, and patient/family needs per established workflows. ### What we're looking for - Active RN license (required); CCM or ACM certification preferred. - Associate's degree in Nursing required; Bachelor's degree preferred. - Minimum 2 years of medical/hospital nursing experience. - Prior care management or utilization management experience preferred. - Demonstrated critical thinking, task prioritization, and customer service skills. ### What's included - Pay range: $35.19–$62.17/hour (~$73,195–$129,314 annually). - Medical, dental, and vision insurance effective Day One. - Paid Time Off from Day One; 4 weeks 100% paid parental leave. - 403(b) retirement plan. - Career development resources and whole-person well-being support. ### About the practice UChicago Medicine AdventHealth is a faith-based health system operating AdventHealth Hinsdale, a community hospital located at 120 N Oak Street in Hinsdale, IL.
