AdventHealth Fish Memorial is hiring a full-time Care Management Supervisor to lead inpatient discharge planning and social services operations at its Orange City, Florida campus. ### What you'll do - Supervise daily discharge planning and social services across inpatient, observation, and emergency department units - Participate in multidisciplinary rounds to drive active staff engagement and timely barrier resolution - Facilitate care conferences for complex transitions and patient placements - Monitor care manager documentation accuracy and regulatory compliance using reports and dashboards - Report quality and risk issues to appropriate departments and committees in a timely manner - Contribute to hiring, onboarding, evaluating, and mentoring care management staff ### What we're looking for - Active Registered Nurse (RN) license in Florida (required) - Associate's degree in Nursing (required); Bachelor's degree preferred - Minimum 2 years of frontline nursing experience (required) - Minimum 1 year of direct care management experience (required) - Working knowledge of CMS Conditions of Participation, medical necessity criteria, IMM, MOON, and Advanced Directives - Accredited Case Manager (ACM) or Certified Case Manager (CCM) credential preferred ### What's included - Salary range: $70,683–$131,480 per year - Medical, dental, vision, life, and disability insurance from day one - Paid time off from day one - 403(b) retirement plan - 4 weeks of fully paid parental leave - Mental health and whole-person well-being resources ### About the practice AdventHealth Fish Memorial is part of AdventHealth, a faith-based not-for-profit health system operating across multiple states.
