About this role

A part-time RN Care Manager role at AdventHealth Fish Memorial in Orange City, Florida, focused on discharge planning and care coordination across the inpatient continuum. ### What you'll do - Participate in multidisciplinary rounds to review patient status, level of care, and discharge plans for all assigned patients - Communicate with and educate patients and families on the emotional, social, and financial impacts of illness while mobilizing community resources - Assess readmitted patients to identify patient- and family-perceived reasons for readmission - Organize and facilitate patient and family care conferences with the multidisciplinary team - Document discharge planning evaluations, ongoing assessments, MDR notes, barriers to progression of care, and avoidable days per standard work ### What we're looking for - Active Florida RN license (required) - Associate's degree in Nursing required; Bachelor's degree preferred - Minimum 2 years of medical/surgical or hospital nursing experience - Knowledge of CMS Conditions of Participation for Discharge Planning - Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred - Prior Care Management or Utilization Management experience preferred ### What's included - Pay range: $32.76–$57.47/hr ($68,141–$119,538 annualized) - Medical, dental, vision, life, and disability insurance from day one - Paid time off from day one - 403(b) retirement plan - 4 weeks of 100% paid parental leave - Mental health resources and whole-person well-being support ### About the practice AdventHealth Fish Memorial is part of AdventHealth, a faith-based, not-for-profit health system operating across multiple states.
