About this role
AdventHealth Dade City is hiring a part-time RN Care Manager (16 hours/week, day shift) at its Dade City, Florida hospital campus.
### What you'll do
- Participate in multidisciplinary rounds to assess patient status, progression of care, and discharge readiness for all assigned patients.
- Conduct holistic discharge planning assessments in inpatient, observation, and emergency department settings, covering prior functioning, support systems, financial, and psychosocial factors.
- Develop and maintain discharge plans with contingency options throughout each hospital stay, arranging post-acute care services and community resources as needed.
- Communicate with payers to obtain authorization for post-acute care services.
- Organize and facilitate patient and family care conferences with the multidisciplinary team.
- Document discharge planning evaluations, multidisciplinary round notes, barriers to progression, and avoidable days in the electronic medical record per standard work.
### What we're looking for
- Active Florida Registered Nurse (RN) license (required).
- Associate's degree in Nursing (required); Bachelor's degree in Nursing (preferred).
- Minimum 2 years of medical/hospital nursing experience (required).
- Prior Care Management or Utilization Management experience (preferred).
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) credential (preferred).
- Knowledge of CMS Conditions of Participation for Discharge Planning and community post-acute care resources.

- Hourly pay range: $32.76–$57.47 (annualized equivalent approximately $68,000–$119,500 based on 40-hour equivalent).
