About this role
AdventHealth Heart of Florida is hiring a full-time Supervisor of Care Management at its Davenport, Florida hospital to lead discharge planning and social services operations across inpatient, observation, and emergency department settings.
### What you'll do
- Supervise daily discharge planning and social services operations, serving as the primary point of contact for inpatient unit care management barriers and concerns.
- Lead and participate in multidisciplinary rounds, ensuring active staff engagement and effective barrier resolution.
- Implement corporate Care Management strategies in collaboration with facility leadership.
- Provide discharge planning and progression-of-care expertise to the interdisciplinary team.
- Monitor care manager documentation accuracy and regulatory compliance using reports and dashboards.
- Apply CMS Conditions of Participation and Value-Based Program guidelines to support compliant, patient-centered care transitions.
### What we're looking for
- Bachelor's degree in Social Work (required); Master's degree preferred.
- Minimum 2 years of social work experience and 1+ year in direct care management.
- Knowledge of medical necessity, patient status, IMM, MOON, Advanced Directives, and CMS discharge planning requirements.
- Ability to lead interdisciplinary teams and drive process improvement initiatives.
- Social work licensure (LBSW, LSW, LMSW, or LCSW) preferred; ACM or CCM certification also preferred.
- Proficiency with PC applications and data reporting tools.
