About this role
AdventHealth Heart of Florida is hiring an Executive Director of Care Management at its Davenport, Florida campus to lead hospital-wide care management operations across acute and post-acute settings.
### What you'll do
- Oversee Acute Care Management, Post-Acute Care Collaborative, and Operations Optimization across market hospital campuses
- Lead clinical care management activities, transitional planning, and progression-of-care strategies to manage length of stay and reduce avoidable readmissions
- Set the program's vision, strategic objectives, and goals in collaboration with clinical and operational leaders
- Monitor Care Management program effectiveness using dashboards and reports, developing strategies to improve outcomes
- Develop and manage the annual operational and capital budget for all areas of responsibility
- Participate in Acute Care Services, Capacity, and Medical Management committees while maintaining current knowledge of federal and regulatory policies
### What we're looking for
- Bachelor's degree in Nursing or a health-related field (required); Master's degree in business or healthcare preferred
- 6+ years of experience in a functional healthcare setting (required)
- 4+ years of progressive leadership experience (required)
- Registered Nurse (RN) licensure preferred
- Strong analytical, strategic planning, and budget management skills
- Demonstrated ability to lead change and communicate effectively across diverse populations in a matrix-management environment
