About this role
AdventHealth Ocala is hiring a part-time RN Care Manager (3 days/week, 8:00 AM–4:30 PM) to support inpatient discharge planning and care transitions at its Ocala, Florida hospital.
### What you'll do
- Participate in multidisciplinary rounds to review patient status, progression, level of care, and discharge plans for all assigned patients.
- Communicate with and educate patients and families on emotional, social, and financial impacts of illness; mobilize community resources accordingly.
- Assess readmitted patients to identify perceived reasons for readmission and develop targeted transition plans.
- Organize and facilitate patient and family care conferences with the multidisciplinary team.
- Document discharge planning evaluations, ongoing assessments, care barriers, avoidable days, and MDR activity in the electronic medical record per standard work.
### What we're looking for
- Active Registered Nurse (RN) license in Florida (required).
- Associate's Degree in Nursing required; Bachelor's of Nursing preferred.
- Minimum 2 years of medical/hospital nursing experience.
- Prior care management or utilization management experience preferred.
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) preferred.
- Proficiency with electronic medical records and strong critical thinking, prioritization, and conflict-resolution skills.
