Care Manager SW

A Care Manager – Social Work position is open at AdventHealth Daytona Beach in Daytona Beach, Florida, supporting inpatient, observation, and emergency department patients through discharge planning and psychosocial care coordination. ### What you'll do - Conduct holistic psychosocial assessments for discharge planning, evaluating prior functioning, support systems, financial status, and social needs across inpatient and ED settings. - Develop and manage individualized discharge and transition-of-care plans, coordinating post-acute care services, community resources, and contingency arrangements throughout the hospital stay. - Participate actively in multidisciplinary rounds to track patient status, progression of care, and discharge readiness, escalating delays to leadership as appropriate. - Provide grief counseling, crisis intervention, disease-adjustment support, and de-escalation services for patients and families. - Communicate with payors to obtain authorization for post-acute services and ensure continuity of care documentation and discharge reconciliation are accurate. - Facilitate family care conferences and advocate for patient rights, informed decision-making, and access to community resources. ### What we're looking for - Master's degree required (social work or related field). - Minimum 2 years of social work experience required. - Strong psychosocial assessment, critical thinking, and interpersonal communication skills. - Proficiency with electronic medical records and standard care management technology platforms. - Accredited Case Manager (ACM) or Certified Case Manager (CCM) credential preferred. - Working knowledge of discharge planning, utilization management, managed care reimbursement, and state/federal care management guidelines preferred. ### What's included - Pay range: $23.71–$44.09/hour (~$49,316–$91,707 annualized). - Medical, dental, vision, life, and disability insurance effective day one. - Paid time off from day one and four weeks of 100% paid parental leave. - 403(b) retirement plan. - Career development resources and whole-person well-being support. ### About the practice AdventHealth Daytona Beach is a faith-based, not-for-profit hospital located at 301 Memorial Medical Pkwy in Daytona Beach, FL, part of the AdventHealth system.
