About this role
A Certified Diabetes Care Coordinator joins AdventHealth's outpatient diabetes self-management education (DSMES) program at the Orlando, Florida campus.
### What you'll do
- Provide diabetes self-management education (DSME) to individual patients, families, and community groups in outpatient settings
- Oversee planning, implementation, and evaluation of the DSMES service, including ADA Recognition maintenance
- Liaise between DSMES team members, other departments, and administration to ensure coordinated program operations
- Monitor team members' qualifications, CE credits, competency, licensures, and registrations
- Assess patient learning needs, develop individualized teaching plans, and document all education services in the permanent education record
- Assist in conducting program evaluations and diabetes management projects in collaboration with other health professionals
### What we're looking for
- Active Registered Dietitian Nutritionist (RDN) or Registered Nurse (RN) license (required)
- Certified Diabetes Care and Education Specialist (CDCES) credential (required)
- Bachelor's degree from an accredited institution (required)
- 2+ years of direct diabetes education experience (required)
- 5+ years of broader healthcare experience (required)
- Strong leadership, communication, and organizational skills
