About this role
AdventHealth Home Health is hiring a full-time Registered Nurse Care Manager to deliver home-based skilled nursing services across Henderson, Polk, Buncombe, and Transylvania Counties in North Carolina.
### What you'll do
- Conduct comprehensive assessments of physical, functional, psychosocial, cognitive, and discharge planning needs for home care patients.
- Develop and implement patient-specific care plans in collaboration with patients, families, and physicians, with measurable individualized goals.
- Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments in patients' homes.
- Review patient history, diagnostic data, and lab results; report abnormal findings to the physician and adjust care plans accordingly.
- Apply motivational interviewing and health coaching techniques to engage patients and families in ongoing care management.
- Maintain accurate clinical records, meeting all documentation deadlines for certifications, re-certifications, and care plan updates.
### What we're looking for
- Active Registered Nurse (RN) license in North Carolina.
- Associate's Degree in Nursing (required); Bachelor's Degree preferred.
- Minimum 1 year of relevant clinical nursing experience.
- Valid driver's license and personal auto insurance (travel to patient homes required).
- Current BLS/CPR certification.
- Experience in a Medicare-certified home health agency as a case manager preferred; COSC certification a plus.

### About the practice
AdventHealth Home Health and Hospice is a faith-based home health organization operating out of Fletcher, NC, focused on extending care into patients' homes and communities. The care management team covers a multi-county territory with a Monday–Friday day shift schedule and a call rotation of approximately 2–3 times per month.
