Director of Care Management RN Lake Nona

AdventHealth Lake Nona is hiring a Director of Care Management RN to lead the care management department at its hospital campus in Orlando, Florida. ### What you'll do - Direct daily operations of the care management department, including discharge planning, care coordination, progression of care, and social services - Oversee recruitment, retention, and leadership development within the care management team - Partner with Utilization Management on the Utilization Review Committee and day-to-day operations - Engage post-acute care entities to improve patient care transitions and reduce readmissions - Manage the operational budget, ensuring capital and fiscal accountability - Educate and mentor care management staff and interdisciplinary teams on data, outcomes, and improvement opportunities ### What we're looking for - Active RN license (required); ACM or CCM certification (preferred) - Bachelor's in Nursing required; Master's in Nursing preferred - 5+ years of direct care management experience - 3+ years of care management leadership experience - 2+ years of frontline nursing experience - Working knowledge of CMS Conditions of Participation for Discharge Planning and managed care/value-based programs ### What's included - Salary range: $94,084–$175,004 annually - Medical, dental, vision, life, and disability insurance from Day One - Paid time off from Day One - 403(b) retirement plan - 4 weeks of 100% paid parental leave - Career development and whole-person well-being resources ### About the practice AdventHealth Lake Nona is part of a comprehensive network of care serving the Orlando, Florida area, with a 60-acre health and wellness campus slated to open in late 2026.
