About this role
AdventHealth Port Charlotte is hiring a PRN Care Manager RN for weekend day shifts at its inpatient facility in Port Charlotte, Florida.
### What you'll do
- Participate in multidisciplinary rounds to assess patient status, level of care, progression, and discharge readiness for all assigned patients.
- Develop and maintain discharge plans with contingency options throughout each hospital stay, arranging post-acute care services, facilities, and community resources.
- Communicate with patients and families regarding the emotional, social, and financial impacts of illness, advocating for their empowerment in health care decisions.
- Coordinate with payors to obtain authorization for post-acute care and ensure continuity of care documentation is complete and accurate.
- Assess readmitted patients to identify perceived reasons for readmission and organize family care conferences with the multidisciplinary team.
- Document discharge planning evaluations, MDR notes, barriers to progression, avoidable days, and transition plans per standard work requirements.
### What we're looking for
- Active Florida Registered Nurse (RN) license (required).
- Associate's degree in Nursing (required); Bachelor's degree in Nursing (preferred).
- Minimum 2 years of medical/hospital nursing experience.
- Prior Care Management or Utilization Management experience preferred.
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) credential preferred.
- Knowledge of CMS Conditions of Participation for Discharge Planning and community post-acute care resources.
### What's included
- Pay range: $32.76–$57.47 per hour (approximately $68,141–$119,538 annualized).
- PRN weekend day shift schedule with flexibility in a dynamic inpatient environment.
