About this role
A Social Work Care Manager role in the Neuro ED Observation unit at AdventHealth Orlando, serving patients and families on night and weekend shifts at the Ginsburg Tower campus in Orlando, Florida.
### What you'll do
- Provide grief counseling, crisis intervention, disease adjustment support, and goals-of-care planning for patients and families in inpatient, observation, and emergency department settings.
- Conduct holistic psychosocial assessments covering prior functioning, support systems, financial factors, and discharge planning needs to prevent care delays.
- Develop and maintain discharge plans with contingency options throughout the hospital stay, arranging post-acute care services and community resources as needed.
- Participate in multidisciplinary rounds to track patient progression, identify discharge resources, and escalate care delays to leadership.
- Communicate with payors regarding authorization for post-acute care and ensure medical records and discharge reconciliation are accurate.
- Facilitate patient and family care conferences, document assessments and barriers in the EMR, and advocate for patient rights and informed decision-making.
### What we're looking for
- Master's degree in Social Work or related field (required).
- Minimum 2 years of social work experience (required).
- 2+ years of care management experience (preferred).
- Strong psychosocial assessment, critical thinking, and interpersonal communication skills.
- Proficiency with electronic medical records and Outlook; ability to work across diverse social, economic, and cultural backgrounds.
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) credential preferred.
