About this role

A Licensed Clinical Social Worker is needed for the Community Care team at AdventHealth Waterman in Tavares, Florida. ### What you'll do - Collaborate with the Community Care team to evaluate patients' medical and psychosocial conditions and develop patient-centered care plans. - Maintain an active caseload of at-risk patients, optimizing their independence through education and community resource connections. - Coordinate care across the continuum to ensure safe, efficient, and cost-effective resource utilization. - Provide crisis intervention and community support resources for patients and families experiencing significant stress related to illness. - Identify social determinants of health affecting patient populations and address health-related needs accordingly. - Assist with community outreach and educational programs related to care coordination. ### What we're looking for - Active LCSW (Licensed Clinical Social Worker) licensure in Florida — required. - Master's degree in Social Work — required. - Valid Driver's License — required. - 3+ years of relevant social work experience — preferred. - Experience in complex ambulatory care and/or population health — preferred. - Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint) and strong written and verbal communication skills. ### What's included - Salary range: $54,230 – $100,876 per year. - Medical, dental, and vision insurance effective from day one. - Paid time off from day one and four weeks of 100% paid parental leave. - 403(b) retirement plan. - Career development resources and whole-person well-being support. ### About the practice AdventHealth Waterman is a faith-based, not-for-profit health system located in Tavares, Florida, serving Lake County and surrounding communities.
