Care Management Assistant

A Care Management Assistant supports patient care coordination at AdventHealth Waterman, a hospital-based health system in Tavares, Florida. ### What you'll do - Coordinate patient care and service arrangements under the direction of an RN or Social Work Care Manager - Arrange patient transportation and follow-up appointments as requested - Obtain insurance authorizations for post-acute care needs as directed by the Care Manager - Deliver and document regulatory letters to patients or their representatives in compliance with Federal and State regulations - Collaborate with Care Managers to address patient care coordination needs - Interact with patients, physicians, and the broader healthcare team to promote quality customer service ### What we're looking for - High school diploma or equivalent (required) - Intermediate to advanced proficiency in Microsoft Office (Word, Outlook, PowerPoint, Excel) - Strong interpersonal and written/verbal communication skills - At least 1 year of office experience, preferably in a medical or clinical setting (preferred) - Prior data entry experience (preferred) - Familiarity with medical terminology (helpful) ### What's included - Pay range: $17.11–$27.38/hour (~$35,589–$56,950 annualized) - Medical, dental, vision, life, and disability insurance from Day One - Paid time off from Day One - 403(b) retirement plan - 4 weeks of 100% paid parental leave - Mental health resources and whole-person well-being support ### About the practice AdventHealth Waterman is part of AdventHealth, a faith-based nonprofit health system operating across multiple states with a mission centered on whole-person care.
