Licensed Practical Nurse LPN Care Management

An LPN Care Management role at AdventHealth is open in Altamonte Springs, Florida, supporting inpatient discharge planning and care coordination across a faith-based health system. ### What you'll do - Collect clinical, social, and financial information from patients and families to guide individualized discharge plans - Collaborate with the RN Care Manager to review medical records and document discharge plans, MDRs, barriers to care progression, and avoidable days - Coordinate post-acute services and facilities, communicating patient care information to ensure continuity of care - Communicate with payors to obtain authorization for post-acute care as needed - Advocate for patient and family rights and empower patients in healthcare decision-making throughout hospitalization - Review patient records for Performance Improvement and Risk Management per established criteria, documenting adverse occurrences ### What we're looking for - Active Licensed Practical Nurse (LPN) licensure in Florida (required) - High school diploma or equivalent plus completion of an accredited LPN program (required) - 2+ years of discharge planning experience (preferred) - 2+ years of LPN acute care experience (preferred) - Strong written and verbal communication skills; ability to collaborate effectively with multidisciplinary care teams - BLS/CPR certification (preferred); proficiency with electronic health records and Microsoft Office ### What's included - Pay range: $20.97–$38.99/hour (~$43,618–$81,099 annualized) - Medical, dental, vision, life, and disability insurance effective Day One - Paid time off from Day One - 403(b) retirement plan - 4 weeks of 100% paid parental leave - Career development resources and whole-person well-being support ### About the practice AdventHealth is a large faith-based, nonprofit health system with facilities across multiple states, rooted in a whole-person care model spanning body, mind, and spirit.
