Registered Nurse Care Management Lake Wales

A full-time Registered Nurse in Care Management at AdventHealth Lake Wales provides inpatient discharge planning and care coordination at the facility on S 11th Street in Lake Wales, Florida. ### What you'll do - Participate in multidisciplinary rounds to review patient status, level of care, and discharge plans, escalating care delays to leadership as needed. - Assess patients and families holistically for discharge planning needs across inpatient, observation, and emergency department settings. - Develop and continuously update discharge plans with contingency options, arranging post-acute care services and community resources throughout the hospital stay. - Communicate with payers to obtain authorization for post-acute care and ensure accurate discharge reconciliation. - Facilitate patient and family care conferences with the multidisciplinary team and educate families on the emotional, social, and financial impacts of illness. - Document discharge planning evaluations, barriers to progression of care, avoidable days, and MDR activity per standard work protocols. ### What we're looking for - Active Registered Nurse (RN) license in Florida (required). - Associate of Nursing degree (required); Bachelor of Nursing preferred. - Minimum 2 years of medical/hospital nursing experience. - Prior care management or utilization management experience preferred. - BLS certification preferred. - CCM or ACM certification preferred. ### What's included - Pay range: $32.16–$56.28/hr ($66,893–$117,062 annualized). - Medical, dental, vision, life, and disability insurance effective Day One. - Paid time off from Day One. - 403(b) retirement plan. - 4 weeks of 100% paid parental leave. - Mental health resources and whole-person well-being support. ### About the practice AdventHealth Lake Wales is a faith-based acute care hospital serving the Lake Wales, Florida community as part of the AdventHealth system.
