Care Management Social Worker Lake Wales

AdventHealth Lake Wales is hiring a full-time Care Management Social Worker to support inpatient, observation, and emergency department patients at its hospital in Lake Wales, Florida. ### What you'll do - Conduct psychosocial assessments for patients and families, covering prior functioning, support systems, financial resources, and discharge planning needs - Develop and maintain discharge plans with contingency options, coordinating post-acute care services, community resources, and care transitions throughout the hospital stay - Participate in multidisciplinary rounds to track patient status, progression of care, and discharge readiness, escalating delays to leadership as needed - Provide grief counseling, crisis intervention, disease-adjustment support, and goals-of-care planning for patients and families - Facilitate patient and family care conferences and communicate with payors to obtain authorizations for post-acute services - Document discharge planning evaluations, ongoing assessments, care barriers, avoidable days, and MDR notes in the electronic medical record per standard work ### What we're looking for - Master's degree in Social Work (required) - Minimum 2 years of social work experience (required) - Strong psychosocial assessment, critical thinking, and interpersonal communication skills - Computer proficiency with Outlook and electronic medical record systems - Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification (preferred) - 2+ years of care management experience and working knowledge of discharge planning, utilization management, and managed care reimbursement (preferred) ### What's included - Pay range: $22.88–$42.54/hour (~$47,590–$88,483 annualized) - Sign-on bonus up to $5,000 (if eligible) - Relocation bonus up to $10,000 (if eligible) - Medical, dental, vision, life, and disability insurance effective Day One - Paid time off from Day One; 4 weeks 100% paid parental leave - 403(b) retirement plan ### About the practice AdventHealth Lake Wales is part of AdventHealth, a faith-based, not-for-profit health system serving communities across the United States with a whole-person care philosophy.
