Director, Care Management

AdventHealth Gordon is hiring a Director of Care Management at its hospital in Calhoun, Georgia, to lead the full care management department including discharge planning, care coordination, and social services. ### What you'll do - Direct daily operations of care management, including discharge planning, care coordination, progression of care, and social services in compliance with CMS and regulatory standards. - Oversee recruitment, retention, and staffing strategies to sustain a high-performing care management team. - Collaborate with interdisciplinary hospital leaders and executives to meet organizational and patient care goals. - Identify improvement opportunities and lead operational, technology, and workflow initiatives aimed at reducing readmissions and improving patient outcomes. - Develop, implement, and monitor care management policies, standard work, and operational strategies across the department. ### What we're looking for - Active Registered Nurse (RN) license required. - Bachelor's degree in Nursing required; Master's in Nursing preferred. - 5+ years of direct care management experience required. - 3+ years of care management leadership experience required. - 2+ years of frontline nursing experience required. - Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification preferred. ### What's included - Salary range: $89,917 – $167,253 annually. - Medical, dental, vision, life, and disability insurance effective Day One. - Paid Time Off from Day One. - 403(b) retirement plan. - Four weeks of 100% paid parental leave. - Mental health resources and whole-person well-being support. ### About the practice AdventHealth Gordon is part of AdventHealth, a faith-based, not-for-profit health system operating across multiple states with a mission rooted in whole-person care.
