Director of Care Management and Social Work

AdventHealth Gordon is hiring a Director of Care Management and Social Work to lead its care management and social services department at its Calhoun, Georgia hospital campus. ### What you'll do - Direct daily operations of care management, including discharge planning, care coordination, progression of care, and social services in compliance with CMS Conditions of Participation and applicable regulations. - Lead recruitment, retention, staffing, and professional development for the care management team. - Collaborate with interdisciplinary hospital leaders, executives, post-acute care partners, and Utilization Management to improve care transitions and reduce readmissions. - Identify and drive operational, workflow, and technology improvement initiatives to enhance departmental performance and patient outcomes. - Develop and implement care management policies, standard operating procedures, and fiscal accountability strategies. ### What we're looking for - Master's in Social Work, or a Bachelor's in Social Work paired with a Master's in a related field (required). - 5+ years of direct care management experience (required). - 3+ years of care management leadership experience (required). - 2+ years of social work experience (required). - Working knowledge of CMS discharge planning CoPs, medical necessity criteria, managed care, and value-based programs. - Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification (preferred). ### What's included - Salary range: $87,270 – $162,324 annually. - Medical, dental, vision, life, and disability insurance from day one. - Paid time off from day one; 4 weeks of 100% paid parental leave. - 403(b) retirement plan. - Career development resources and whole-person well-being support. ### About the practice AdventHealth Gordon is part of the AdventHealth system, a faith-based, not-for-profit health network with a mission rooted in whole-person care. Located in Calhoun, Georgia, the facility serves the surrounding northwest Georgia community with a full range of inpatient and outpatient services. The care management department operates Monday through Friday, 8:30 a.m. to 5:00 p.m., within a collaborative, interdisciplinary leadership structure.
