About this role

AdventHealth South Overland Park is hiring a full-time RN Care Management to coordinate inpatient discharge planning and care transitions at its hospital in Overland Park, Kansas. ### What you'll do - Participate in multidisciplinary rounds to review patient status, level of care, and discharge plans, escalating care delays to leadership as needed. - Assess patients and families holistically for discharge planning needs across inpatient, observation, and emergency department settings, incorporating clinical, social, and financial factors. - Develop and maintain discharge plans with contingency options throughout the hospital stay, arranging post-acute care services, facilities, and community resources. - Facilitate patient and family care conferences with the multidisciplinary team and communicate with payors regarding authorization for post-acute services. - Document discharge planning evaluations, MDR participation, barriers to progression of care, and avoidable days per standard work protocols. - Leverage technology and best practices to communicate with post-acute providers, ensure medical record completeness, and confirm accurate discharge reconciliation. ### What we're looking for - Active Registered Nurse (RN) license required. - Associate's degree in Nursing required; Bachelor's degree preferred. - Minimum 2 years of medical/hospital nursing experience required. - Prior Care Management or Utilization Management experience preferred. - Working knowledge of CMS Conditions of Participation for Discharge Planning and community post-acute resources. - Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred. ### What's included - Pay range: $33.70–$62.69/hour ($70,096–$130,395 annualized). - Medical, dental, vision, life, and disability insurance effective Day One. - Paid Time Off from Day One; 4 weeks fully paid parental leave. - 403(b) retirement plan. - Career development support and whole-person well-being resources. ### About the practice AdventHealth South Overland Park is part of the AdventHealth system, a faith-based not-for-profit health network focused on whole-person care.
