Registered Nurse RN Case Manager Emergency Department

UChicago Medicine AdventHealth is hiring a full-time Registered Nurse Case Manager for the Emergency Department at its Bolingbrook, Illinois campus. ### What you'll do - Participate in multidisciplinary rounds to review patient status, progression of care, and discharge planning for all assigned ED and observation patients. - Assess patients and families holistically for discharge planning needs, including prior functioning, support systems, financial, and psychosocial factors. - Develop and maintain discharge plans with contingency options throughout the hospital stay, arranging post-acute care services and community resources as needed. - Communicate with payors to obtain authorization for post-acute care and ensure continuity of clinical information across care transitions. - Organize and facilitate patient and family care conferences with the multidisciplinary team, educating families on the emotional, social, and financial impacts of illness. - Document discharge planning evaluations, barriers to progression, avoidable days, and MDR participation in the electronic medical record per standard work. ### What we're looking for - Active Registered Nurse (RN) license in Illinois (required). - Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred. - Minimum 2 years of medical/surgical or hospital nursing experience. - Prior care management or utilization management experience preferred. - Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred. - Knowledge of CMS Conditions of Participation for Discharge Planning and community post-acute care resources. ### What's included - Pay range: $35.19–$62.17/hour (approximately $73,195–$129,314 annualized). - Medical, dental, vision, life, and disability insurance effective Day One. - Paid time off from Day One; 4 weeks of 100% paid parental leave. - 403(b) retirement plan. - Career development resources and whole-person well-being support. ### About the practice UChicago Medicine AdventHealth Bolingbrook is a faith-based community hospital located at 500 Remington Blvd in Bolingbrook, Illinois, serving the southwestern suburbs of Chicago. The facility emphasizes whole-person care and operates as part of the broader AdventHealth system, combining academic medicine resources with community-level access. The Case Manager role works a 36-hour week on day shifts (8a–8:30p), every other Saturday and rotating holidays.
