About this role
An RN Transition Specialist – Division Team Float at AdventHealth supports high-risk patients through safe care transitions at the Courtland Campus in Orlando, Florida.
### What you'll do
- Identify patients with moderate to high-risk readmission risk and collaborate with the treatment team to coordinate safe, effective transitions of care.
- Assess, educate, and intervene with patients and families on disease self-management during hospitalization and for 30–90 days post-discharge.
- Evaluate medication adherence, review regimens, and deliver targeted education to improve patient compliance.
- Arrange post-acute resources and community support for patients who require additional services after discharge.
- Apply the full nursing process—assessment, planning, implementation, and evaluation—to meet individualized patient goals.
- Function as a patient advocate, navigating complex internal and external healthcare systems on behalf of patients and families.
### What we're looking for
- Active Registered Nurse (RN) license in Florida (required).
- Minimum 1 year of nursing experience; 2+ years in care management, chronic disease management, or care coordination (required).
- Associate's degree in Nursing (required); BSN or MSN preferred.
- Knowledge of chronic disease management including heart failure, COPD, pneumonia, and myocardial infarction.
- Proficiency in Microsoft Word, Excel, and Windows; Cerner experience preferred.
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification preferred.
