An RN Care Manager role at AdventHealth in Sebring, Florida, coordinating inpatient discharge planning and transitions of care across a hospital-based multidisciplinary team.
### What you'll do
- Participate in daily multidisciplinary rounds to review patient status, level of care, and discharge readiness, escalating care delays as needed.
- Conduct holistic discharge planning assessments in inpatient, observation, and emergency settings, covering functional, psychosocial, financial, and support-system factors.
- Communicate with payers to obtain authorizations for post-acute care services and coordinate placement with post-acute facilities and community resources.
- Educate patients and families on the social, emotional, and financial impacts of illness, advocating for their empowerment in healthcare decisions.
- Document discharge planning evaluations, MDR participation, barriers to progression, avoidable days, and transition-of-care plans per standard work protocols.
- Organize and facilitate patient and family care conferences with the multidisciplinary team, including follow-up on readmitted patients to identify root causes.
### What we're looking for
- Active Florida RN license (required).
- Associate's degree in Nursing (required); Bachelor's degree preferred.
- Minimum 2 years of medical/hospital nursing experience.
- Prior care management or utilization management experience preferred.
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) credential preferred.
### What's included
- Pay range: $32.16–$56.28/hr (~$66,893–$117,062 annually).
