About this role
AdventHealth Orlando is hiring a full-time Registered Nurse Care Manager for the Surgical Transplant unit at its flagship Orlando, Florida campus.
### What you'll do
- Participate in multidisciplinary rounds to review patient status, progression of care, and discharge plans, escalating delays to leadership as appropriate.
- Assess patients and families holistically for discharge planning needs — including prior functioning, support systems, financial, and psychosocial factors — across inpatient, observation, and emergency settings.
- Develop and maintain discharge plans with contingency options throughout the hospital stay, arranging post-acute care services, facilities, and community resources.
- Communicate with payors to obtain authorization for post-acute care and ensure continuity through accurate discharge reconciliation.
- Organize and facilitate patient and family care conferences with the multidisciplinary team.
- Document discharge planning evaluations, ongoing assessments, barriers to progression, avoidable days, and MDR outcomes per standard work protocols.
### What we're looking for
- Active Registered Nurse (RN) license in Florida — required.
- Associate of Nursing degree — required; Bachelor of Nursing — preferred.
- Minimum 2 years of medical/hospital nursing experience — required.
- Prior care management or utilization management experience — preferred.
- BLS certification — preferred; CCM or ACM certification — preferred.
- Strong critical thinking, conflict resolution, and organizational skills with proficiency in electronic medical records.

- Hourly pay range: $32.76 – $57.47 (annualized approximately $68,141 – $119,538).
