About this role
AdventHealth Celebration is hiring a full-time Care Management Assistant for its care coordination team at its hospital campus in Celebration, Florida.
### What you'll do
- Coordinate patient care and service arrangements under the direction of an RN or Social Work Care Manager, in collaboration with patients and families.
- Deliver and document regulatory letters to patients or their representatives in compliance with Federal and State regulations.
- Arrange patient transportation and follow-up appointments as requested by the Care Manager.
- Obtain insurance authorizations for post-acute care needs as directed.
- Collaborate with Care Managers, physicians, and the broader healthcare team to support patient care coordination.
### What we're looking for
- High school diploma or equivalent required.
- Intermediate to advanced proficiency in Microsoft Office (Word, Outlook, PowerPoint, Excel).
- Strong interpersonal and written/verbal communication skills.
- At least 1 year of office experience, preferably in a medical or healthcare setting (preferred).
- Previous data entry experience preferred.
- Familiarity with medical terminology is a plus.
