Care Management Social Worker FT Nonexempt

AdventHealth Celebration is hiring a full-time Care Management Social Worker to support inpatient, observation, and emergency department patients at its Celebration, Florida hospital. ### What you'll do - Provide grief counseling, disease adjustment support, crisis intervention, and goals-of-care planning for patients and families. - Conduct holistic psychosocial assessments addressing prior functioning, support systems, financial needs, and discharge planning to prevent care delays. - Develop and maintain discharge plans with contingency options throughout each hospital stay, coordinating post-acute care services and community resources. - Participate actively in multidisciplinary rounds to review patient status, progression of care, and discharge readiness, escalating barriers to leadership as appropriate. - Communicate with payers to obtain authorization for post-acute care services and ensure medical records and discharge reconciliation are complete. - Organize and facilitate patient and family care conferences, documenting assessments, discharge plans, and barriers according to department standard work. ### What we're looking for - Master's degree in Social Work or related field (required). - Minimum 2 years of social work experience (required); 2+ years in care management (preferred). - Strong psychosocial assessment, critical thinking, and interpersonal communication skills. - Proficiency with electronic medical records and working knowledge of pre- and post-acute care venues and community resources. - Accredited Case Manager (ACM) or Certified Case Manager (CCM) credential (preferred). - Current knowledge of discharge planning, utilization management, and managed care reimbursement (preferred). ### What's included - Pay range: $23.71–$44.09/hour (~$49,316–$91,707 annualized). - Medical, dental, vision, life, and disability insurance effective Day One. - Paid time off from Day One and 4 weeks of 100% paid parental leave. - 403(b) retirement plan. - Career development resources and whole-person well-being support. ### About the practice AdventHealth Celebration is a full-service acute care hospital located in Celebration, Florida, part of the AdventHealth system — one of the largest faith-based health networks in the United States.
