A Discharge Transition Specialist LPN at AdventHealth Ocala supports seamless care transitions for inpatient, outpatient, and emergency department patients in Ocala, Florida.
### What you'll do
- Collect clinical, social, and financial information from patients and families to inform individualized discharge planning
- Collaborate with RN Care Managers to review medical records and coordinate post-acute services and facilities
- Communicate with payors to obtain authorizations for post-acute care as needed
- Document discharge plans, multidisciplinary rounds (MDRs), barriers to progression of care, avoidable days, and patient/family needs per standard work
- Advocate for patient and family rights, empowering informed healthcare decisions throughout hospitalization
- Leverage technology and best practices to ensure medical records are complete, care information is communicated to post-acute providers, and discharge reconciliation is accurate
### What we're looking for
- Active Florida Licensed Practical Nurse (LPN) license required
- Minimum 2 years of discharge planning experience
- Minimum 2 years of LPN acute care experience
- High school diploma or equivalent plus completion of an LPN vocational/technical program
- Strong documentation and communication skills; ability to collaborate across interdisciplinary care teams
- Basic Life Support (BLS/CPR) certification preferred
