TeamHealth is hiring a Nurse Practitioner for a post-acute care role serving skilled nursing and long-term care facilities in Southwest Dallas, TX.
### What you'll do
- Provide clinical care to patients in post-acute and long-term care settings across the Southwest Dallas area
- Perform comprehensive assessments, develop care plans, and manage chronic and acute conditions
- Collaborate with facility staff, attending physicians, and interdisciplinary care teams
- Document clinical encounters and maintain accurate medical records in compliance with regulatory standards
- Respond to changes in patient condition and coordinate transitions of care
### What we're looking for
- Active Nurse Practitioner license in the state of Texas
- Board certification as a Family, Adult, Adult-Gerontology, or Gerontology NP
- Experience in post-acute, long-term care, or related clinical setting preferred
- Ability to manage an independent patient panel with physician collaboration
- Strong communication and documentation skills
