About this role
AdventHealth is hiring a Complex Case Licensed Clinical Social Worker (LCSW) at its Greenwood Village, Colorado facility on S. Willow Drive.
### What you'll do
- Conduct comprehensive psychosocial assessments for inpatient, observation, and emergency department patients to identify discharge planning needs including prior functioning, support systems, and financial factors.
- Develop and manage discharge plans with contingency options throughout the hospital stay, arranging post-acute care services, community resources, and continuity of care documentation.
- Participate actively in multidisciplinary rounds to review patient status, progression of care, and discharge readiness, escalating delays to leadership as needed.
- Provide grief counseling, crisis intervention, disease adjustment support, and goals-of-care planning for patients and families.
- Facilitate patient and family care conferences with the interdisciplinary team and advocate for patient rights and self-determination.
- Communicate with payors regarding authorization for post-acute care and ensure accurate discharge reconciliation.
### What we're looking for
- Master's in Social Work (MSW) required.
- Active LCSW licensure in Colorado (additional licensure requirements may apply per unit).
- Minimum 4 years of social work experience required.
- Experience in care management preferred.
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification preferred.
- Demonstrated proficiency in psychosocial assessment across the full age continuum, from newborn to geriatric.
