About this role
AdventHealth Lake Wales is hiring a full-time Discharge Callback RN to support post-discharge patient follow-up and care continuity at its Lake Wales, Florida facility.
### What you'll do
- Place outbound calls to discharged patients, making up to three contact attempts per patient to achieve 100% callback coverage.
- Follow scripted protocols to gather consistent, accurate post-discharge feedback.
- Identify and escalate priority clinical or safety concerns discovered during calls to the appropriate resource.
- Educate patients on the HCAHPS survey process to support patient satisfaction goals.
- Maintain accurate call log data and generate call log reports for leadership review.
- Coordinate with directors, guest relations, physicians, and home health services to ensure timely resolution of patient needs.
### What we're looking for
- Active Florida Registered Nurse (RN) license (required).
- Associate's Degree in Nursing (required); Bachelor's Degree in Nursing preferred.
- Current BLS/CPR certification (required).
- Proficiency in Cerner and strong data entry/keyboard skills (required).
- Effective written and verbal communication in English; additional languages preferred.
- Working knowledge of customer service principles and practices.
