AdventHealth Parker is hiring a full-time Manager of Care Management to lead the Care Management department at its inpatient hospital in Parker, Colorado.
### What you'll do
- Oversee daily operations of Care Management across inpatient, observation, and emergency department settings
- Supervise care managers delivering end-to-end discharge planning from admission through discharge to reduce delays and readmissions
- Participate in and ensure staff presence at daily multidisciplinary rounds to optimize interdisciplinary care coordination
- Collaborate with hospital leadership to escalate pace-of-care delays and drive throughput improvements
- Manage staff escalations for complex cases, coordinating with internal and external care providers to minimize unnecessary patient days
- Lead hiring, onboarding, orientation, and ongoing competency development for the care management team
### What we're looking for
- Active Registered Nurse (RN) license — required
- Associate's Degree in Nursing — required; Bachelor's Degree in Nursing — preferred
- 3+ years of direct care management experience — required
- 2+ years of frontline nursing experience — required
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification — preferred
- Demonstrated ability to lead multidisciplinary teams and drive process improvement initiatives
