About this role

AdventHealth South Orlando is hiring a full-time Registered Nurse for its Care Management team in the Surgical Transplant unit, located at 601 E Rollins St, Orlando, Florida. ### What you'll do - Participate in multidisciplinary rounds to review patient status, progression of care, level of care, and discharge planning for all assigned patients. - Assess patients and families holistically for discharge planning needs, including prior functioning, support systems, financial circumstances, and psychosocial factors. - Develop and maintain discharge plans with contingency options throughout the hospital stay, arranging post-acute care services, facilities, and community resources. - Communicate with payors regarding authorization for post-acute care and ensure medical records and discharge reconciliation are complete and accurate. - Facilitate patient and family care conferences with the multidisciplinary team, advocating for patient empowerment in healthcare decisions. - Document discharge planning evaluations, MDR notes, barriers to progression, avoidable days, and transition-of-care plans in the electronic medical record per standard work. ### What we're looking for - Associate's degree in Nursing (required); Bachelor's degree in Nursing (preferred). - Active Registered Nurse (RN) licensure (required). - Minimum 2 years of medical/hospital nursing experience (required). - Prior care management or utilization management experience (preferred). - Certified Case Manager (CCM) or Accredited Case Manager (ACM) credential (preferred). - Strong critical thinking, conflict resolution, and organizational skills with proficiency in electronic medical records. ### What's included - Pay range: $32.76–$57.47/hour (~$68,141–$119,538 annualized). - Medical, dental, vision, life, and disability insurance effective Day One. - Paid time off from Day One and 4 weeks of 100% paid parental leave. - 403(b) retirement plan. - Career development programs and whole-person well-being resources, including mental health support. ### About the practice AdventHealth South Orlando is part of AdventHealth, a faith-based, not-for-profit health system with a mission centered on whole-person care — body, mind, and spirit.
