Practice Highlights

- Social Worker leads a defined patient population by service line and serves as a resource to team members on Tenet performance standards
- Conducts complex psychosocial transition planning assessments and reassessments, including identification of patients at risk for readmission
- Facilitates multidisciplinary patient care conferences, including Complex Case Review and TEMPO facilitation as needed
- Manages adoption, abuse, and neglect cases — including assessment, intervention, and referral to local, state, and/or federal agencies
- Collaborates directly with physicians, office staff, ancillary departments, patients, and families on individualized plans of care
- Participates in department quality improvement initiatives and precepts new staff members
- Documents in the Case Management system with timeliness and accuracy, including maintenance of patient demographic and insurance information
Qualifications
- Associate of Science (A.S.) degree required
- Current California license as an Associate Clinical Social Worker (ACSW) required
- Accredited Case Manager (ACM) certification preferred
- Minimum two years of acute hospital social work or case management experience preferred
- Demonstrated competency in psychosocial assessment, transition planning, care coordination, and post-acute discharge planning
- Familiarity with state and federal regulatory requirements and TJC accreditation standards preferred
