Practice Highlights

  • Full-time, day-shift position embedded within the Case Management department of a 209-bed acute care hospital

  • Collaborative interdisciplinary team model — daily coordination with RN Clinical Care Coordinators, physicians, nursing staff, rehabilitation team, pharmacy, and laboratory

  • Conducts facility-based, telephonic, and home assessments; develops individualized care plans and provides ongoing outreach to patients and families
  • Provides abuse detection and mandatory reporting, crisis counseling, and coordination of family and multidisciplinary team conferences
  • Advises patients on Medicare and Medicaid coordination of benefits, assists with Medicaid eligibility maintenance, and applies financial information to support cost-effective, quality care planning
  • Maintains accurate clinical documentation including condition codes, intervention records, and outcome reporting in accordance with Case Management Department policy
  • Hospital service lines include 24-hour emergency care, the region's only heart attack receiving center (between Modesto and Fresno), the Emanuel Cancer Center, an advanced center for mothers and newborns, rehabilitation and therapy, orthopedics, and advanced stroke treatment
  • Part of the Central Valley Doctors Health System, offering institutional resources and collegial professional network

Qualifications

  • Licensed Master of Social Work (MSW) from an accredited program, or current Licensed Clinical Social Worker (LCSW) licensure in California
  • Minimum two years of experience in an acute care hospital and/or skilled nursing facility setting
  • Demonstrated ability to function effectively within a multidisciplinary team environment
  • Working knowledge of the physical, mental, and social needs of elderly patients
  • Availability to rotate weekend coverage as part of the standard schedule
