Practice Highlights

  • Performs discharge planning assessments for Medicare, Medicaid, commercial, private pay, and high-risk patients across multiple inpatient units
  • Conducts utilization review with working knowledge of Medicare, Medicaid, HMO, and private insurance coverage criteria
  • Coordinates post-hospital care arrangements with a goal of completing home care and placement plans 24 hours prior to a known discharge date
  • Communicates directly with attending physicians to facilitate post-acute care transitions and support clinical decision-making
  • Maintains ongoing dialogue with patients, families, and caregivers throughout the hospitalization to assess and address evolving discharge needs
  • Collaborates daily with admissions personnel to track admissions and discharges across units
  • Partners with the Business Office Manager and on-site eligibility worker to connect patients with financial assistance resources
  • Maintains a current working knowledge of community resources and distributes appropriate resource materials to patients and families
  • Active participant in Utilization Management Committee, CQI meetings, and interdisciplinary team meetings
  • Serves a broad outpatient and inpatient catchment area encompassing Sunnyvale, Forney, Mesquite, Seagoville, and surrounding communities
  • Facility features an extensive outpatient services footprint: physical medicine, wound care, sleep center, diagnostic imaging, and two free-standing Emergency Departments

Qualifications

  • Current Registered Nurse (RN) licensure in the State of Texas required
  • Working knowledge of Medicare, Medicaid, HMO, and private insurance coverage criteria and utilization review processes
  • Clinical experience in direct patient care; experience in case management or discharge planning strongly preferred
  • Demonstrated ability to assess patient financial status, diagnosis, and post-acute care needs and match patients to appropriate resources and care settings
  • Strong written and verbal communication skills in English; proficiency in additional languages is a plus
  • Basic computer proficiency; ability to maintain thorough, timely documentation consistent with current standards and policies
  • Organized, self-directed, and able to manage a complex workload under time-sensitive conditions
