This role carries full accountability for medical necessity review, transition planning, and readmission prevention across an acute care campus with nationally recognized cardiac, stroke, neurosurgery, orthopedics, and robotic surgery programs. - Documentation managed within a dedicated Case Management information system; responsibilities include tracking potentially avoidable days and identifying over- and underutilization
- Practice standards aligned with TJC accreditation requirements and state and federal regulatory frameworks
Qualifications
- Active Registered Nurse (RN) license required; Florida licensure or eligibility to obtain
- Minimum two years of acute hospital patient care experience required
- BSN preferred
- Certification as Accredited Case Manager (ACM) or equivalent preferred; acute hospital case management experience preferred
- Current BLS certification required
- Demonstrated organizational skills with strong verbal and written communication; ability to lead and coordinate a diverse multidisciplinary team in a high-acuity environment
- Critical thinking, problem-solving aptitude, and computer literacy required
