Practice Highlights

  • Facilitates care coordination across the continuum — from admission through safe, timely discharge — with a focus on readmission prevention and appropriate level-of-care placement
  • Conducts complex psychosocial assessments and targeted interventions to promote throughput efficiency and patient satisfaction
  • Identifies and stratifies patients at risk for readmission, developing individualized transition plans in collaboration with the clinical team
  • Integrates national case management standards across four core domains: Transition Management, Care Coordination, Regulatory Compliance (state, federal, TJC), and Patient/Family/Caregiver Education
  • Ensures compliance with The Joint Commission (TJC) accreditation standards and applicable state and federal regulatory requirements
  • Collaborates with physicians, nurses, and ancillary staff to sequence care appropriately and optimize length of stay
  • Part of the Baptist Health System — a regional network encompassing six full-service hospitals, a specialized children's hospital with a dedicated pediatric emergency unit, a comprehensive cancer care network, imaging centers, ambulatory services, fitness and rehabilitation centers, and the Baptist School of Health Professions

Qualifications

  • Minimum: Bachelor of Social Work (BSW) from an accredited program; Master of Social Work (MSW) preferred
  • Current Texas State Social Work License commensurate with degree level (required)
  • Accredited Case Manager (ACM) certification preferred
  • No prior experience required; two or more years of acute care social work experience preferred
  • Demonstrated ability to perform complex psychosocial assessments and manage discharge planning in a high-acuity inpatient setting
