Saint Luke's Baptist Hospital in San Antonio, Texas — part of a six-hospital health system with more than 120 years of community care history — is recruiting a licensed Master's-level Social Worker (Social Worker I) to join its inpatient care coordination team. Practice Highlights

  • Inpatient social work role embedded within a large, multi-hospital health system — six full-service hospitals plus a dedicated children's hospital with a pediatric emergency unit
  • Conducts complex psychosocial assessments and targeted interventions to promote throughput efficiency, safe discharge planning, and readmission prevention
  • Manages transition care along the full continuum: appropriate length-of-stay stewardship, level-of-care alignment, and post-acute coordination
  • Identifies and stratifies patients at risk for readmission; develops individualized transition plans in collaboration with the interdisciplinary team
  • Provides education to physicians, patients, families, and caregivers; delivers in-service training for hospital staff
  • Collaborates with community providers to develop and maintain educational resources for staff and patient populations
  • Operates within a compliance framework aligned with state and federal regulatory requirements, The Joint Commission (TJC) accreditation standards, and system policy
  • Participates in planning the social work component for selected hospital programs and contributes to system-wide care coordination initiatives

Qualifications

  • Master of Social Work (MSW) degree required
  • Minimum two years of experience in an acute hospital setting
  • Current Texas State Social Work License commensurate with degree level (LMSW or LCSW)
  • Accredited Case Manager (ACM) certification required
  • Demonstrated ability to work autonomously and exercise independent clinical judgment in a fast-paced inpatient environment
