A multispecialty physician group serving the greater Phoenix West Valley is recruiting a Patient Navigator Registered Nurse to join its care coordination team in Goodyear, Arizona. This role sits within a large integrated health system comprising approximately 20 primary care and specialty outpatient clinic locations, five acute care hospitals, and four neighborhood emergency centers — giving the right RN broad exposure to complex, multi-site patient populations. The core focus is optimizing patient flow and continuity of care by serving as the primary clinical liaison between patients, families, and the broader healthcare team.

Qualifications

  • Active Arizona Registered Nurse (RN) license required
  • Bachelor of Science in Nursing (BSN) required; Master's degree in Informatics or a related healthcare field preferred
  • Minimum one year of nursing experience required
  • American Heart Association (AHA) Basic Life Support (BLS) certification required
