<h3>Practice Highlights</h3> <ul> <li>Hospital-based case management role embedded within an acute care facility serving the Rio Grande Valley region</li> <li>Scope integrates four core domains: Utilization Management, Transition Management, Care Coordination, and Regulatory Compliance</li> <li>Performs accurate medical necessity screening and submits cases for Physician Advisor review; manages concurrent payer disputes</li> <li>Leads and facilitates multidisciplinary patient care conferences and Complex Case Reviews</li> <li>Oversees full transition planning cycle — assessment, reassessment, implementation, and post-acute coordination — to reduce avoidable readmissions and length of stay</li> <li>Identifies and refers complex psychosocial cases to Social Work Services; collaborates with physicians, office staff, and ancillary departments</li> <li>Ensures compliance with state and federal regulatory requirements, The Joint Commission (TJC) accreditation standards, and institutional policy</li> <li>Documents in the Case Management system with timeliness and precision; tracks potentially avoidable days and reports over- and underutilization</li> <li>Provides education to patients, families, caregivers, and physicians to support post-acute care needs and safe discharge planning</li> <li>Operates within a mission-driven health system with a continuous presence in the community since the early 1920s</li> </ul>

<h3>Qualifications</h3> <ul> <li>Active RN license in the state of Texas (or eligibility to obtain); must meet all state law and regulatory requirements for practice</li> <li>Graduate of an accredited school of nursing required; bachelor's or master's degree in nursing preferred</li> <li>Minimum 2 years of acute hospital patient care experience required</li> <li>Prior acute hospital case management experience strongly preferred</li> <li>Accredited Case Manager (ACM) certification preferred</li> </ul>
