- Day-shift role focused on transition management, care coordination, and readmission prevention across a high-acuity inpatient population
- Leads and facilitates multidisciplinary patient care conferences, including Complex Case Review
- Manages adoption, abuse, and neglect cases with appropriate referral to local, state, and federal agencies
- Collaborates with physicians, office staff, ancillary departments, and community providers to develop patient and staff educational resources
- Documentation conducted within Tenet's proprietary Case Management documentation system
- Practice standards aligned with TJC accreditation requirements and state and federal regulatory compliance
Qualifications
- Master of Social Work (MSW) required
- Active Florida social work license, certification, or registration required (as mandated by state law and regulation)
- Accredited Case Manager (ACM) credential preferred
- Minimum of two years of acute hospital social work experience preferred
- Demonstrated organizational skills with the ability to lead and coordinate a diverse, multidisciplinary team in a fast-paced environment
- Strong verbal and written communication skills; critical thinking, problem-solving ability, and computer literacy required
