Fort Mill Medical Center — a 100-bed acute care facility that opened in 2022 in Fort Mill, SC — is recruiting a decentralized, unit-based Clinical Pharmacist to serve as an embedded clinical resource across its ICU, medical/surgical, and women's care service lines. This is a hands-on, patient-facing role with direct rounding responsibilities alongside Intensivists and Hospitalists, meaningful protocol-driven dosing authority, and the full scope of clinical pharmacy practice in a brand-new, technology-forward hospital environment.

Practice Highlights
- Decentralized, unit-based model — pharmacist is physically present in patient care areas, not centralized in a dispensing role
- Active rounding with Intensivists and Hospitalists; pharmacist makes real-time therapy recommendations and participates in care-team decision-making
- Aminoglycoside and vancomycin dosing and monitoring for all hospital patients
- Coumadin dosing and monitoring for all consulted patients
- TPN order writing for all adult and pediatric (non-neonate) patients, with coordination across physician and dietary teams
- IV-to-PO conversion management, renal-function-based dose adjustments, and auto-substitutions per institutional policy
- Medication reconciliation and discharge medication review as standard workflow
- Drug information resource and education for physicians, nurses, and patients
- 100-bed facility with a 24/7 emergency department, intensive care unit, medical/surgical unit, and comprehensive women's care including The Nest labor and delivery suite

Qualifications
- Doctor of Pharmacy (PharmD) with completed clinical rotations; OR Bachelor of Science in Pharmacy with a minimum of 5 years of clinical pharmacist experience
- Active South Carolina pharmacist license required at time of hire
- American Heart Association BLS certification required — must be obtained within 7 days of hire
- ACLS certification required — must be obtained within the orientation period
- Residency training and board certification (BPS or equivalent) preferred
- Comfort with protocol-driven dosing (aminoglycosides, vancomycin, anticoagulation, TPN) and collaborative rounding in an ICU/hospitalist environment
