Practice Highlights

  • 217-bed acute care hospital with recognized service lines in cardiovascular care, orthopedics, obstetrics, and emergency medicine
  • Rotating mid/evening schedule: shifts run 10:00 AM–8:00 PM or 12:00 PM–10:00 PM
  • Weekend rotation: every third weekend, 12:00 PM–10:00 PM
  • On-call coverage as assigned per department policy
  • Collaborative practice management under facility-approved protocols covering anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside dosing consults
  • Clinical scope includes renal dose adjustments, IV-to-PO conversions, high-risk IV verification, black box warning interventions, and therapy initiation/modification/discontinuation
  • Order verification with full medication profile review: duplicate therapy, drug–drug and drug–food interactions, allergy screening, dosing, and frequency checks
  • Medication reconciliation support and clarification of unsafe or unclear drug therapy orders
  • Adverse drug reaction reporting and participation in drug utilization review and department quality improvement initiatives
  • Education and training responsibilities for physicians, nurses, pharmacy technicians, and pharmacy students
  • Supervision of pharmacy technicians in preparation, maintenance, and dispensing functions
  • Regulatory compliance duties including nursing unit inspections and monthly perpetual inventory checks
  • May perform medication management activities at a remote site commonly owned and operated by the hospital, under supervision of the facility DEA registrant or physician lead

Qualifications

  • Graduate of an accredited School of Pharmacy (Pharm.D. or equivalent)
  • Current Pharmacist license in the State of Arizona (or eligibility to obtain)
  • One year of acute care hospital pharmacy experience preferred; new graduates will be considered
  • Board Certification in Pharmacotherapy (BCPS, CGP, or equivalent) preferred but not required
  • Proficiency in a fast-paced, high-pressure inpatient environment with strong prioritization skills
  • Basic computer skills; effective interpersonal and customer service skills; ability to work across all areas of pharmacy services as assigned
