Westover Hills Baptist Hospital in San Antonio, Texas is seeking a licensed Master's-level Social Worker to join its Case Management team in a full-scope, clinically complex role. This position sits within Baptist Health System — a 120-plus-year institution comprising six full-service hospitals, a specialized children's hospital, a comprehensive cancer care network, imaging centers, and ambulatory services across the region. The Social Worker II functions as a central coordinator of patient transitions, psychosocial care, and discharge planning, operating with a high degree of clinical autonomy in an acute inpatient environment. Practice Highlights

  • Acute inpatient case management role with full responsibility for patient transition planning across the care continuum
  • Conducts complex psychosocial assessments and targeted interventions to promote safe, timely discharge and prevent avoidable readmissions
  • Identifies and stratifies patients at risk for readmission; develops and executes individualized transition plans
  • Integrates national case management standards across four core domains: Transition Management, Care Coordination, Regulatory Compliance, and Patient/Family/Caregiver Education
  • Collaborates with physicians, nursing staff, patients, families, and community-based providers to ensure appropriate level-of-care placement and resource utilization
  • Participates in planning the social work component for selected hospital programs and delivers in-service education to clinical staff
  • Develops community-facing educational resources in partnership with external providers
  • Operates within a system that includes six full-service hospitals, a dedicated pediatric hospital with its own emergency unit, a cancer care network, rehabilitation centers, imaging centers, and the Baptist School of Health Professions
  • Works in compliance with TJC accreditation standards, state and federal regulatory requirements, and system-level policy

Qualifications

  • Master of Social Work (MSW) degree required
  • Minimum two years of experience in an acute hospital setting
  • Current Texas State Social Work License commensurate with degree level, required at time of hire
  • Demonstrated ability to work autonomously and exercise independent clinical judgment
  • Accredited Case Manager (ACM) certification preferred
