- Scope includes adoption cases, abuse and neglect assessment and intervention, referrals to local, state, and federal agencies, and coordination with patients and families on the full plan of care
- Responsibility for identifying and managing patients at elevated risk for readmission, with direct oversight or implementation of individualized transition plans
- Affiliated with Piedmont Medical Center – Fort Mill (opened 2022), a 100-bed campus with 24/7 ED, ICU, Medical/Surgical Unit, Surgical Services, and Women's Services — expanding the regional care network
Qualifications
- Master of Social Work (MSW) degree required
- Active South Carolina social work license required; must meet all state licensure, certification, or registration requirements applicable to the state of practice
- Minimum two years of acute hospital social work experience preferred
- Accredited Case Manager (ACM) credential preferred
- Demonstrated competency in complex psychosocial assessment, care coordination, and multidisciplinary collaboration in an inpatient setting
