Practice Highlights
- Provide attending physician services to a SNF resident panel of approximately 75–125 patients
- Full-time schedule, 4–5 days per week
- Conduct admission H&Ps within 48–72 hours, monthly recertifications, and significant-change-of-condition visits
- Manage chronic disease, post-acute recovery, wound care, dementia care, polypharmacy, and end-of-life care
- Lead and collaborate with NPs, PAs, nursing staff, therapy teams (PT/OT/SLP), social work, and consultant pharmacy
- Participate in interdisciplinary care planning, family conferences, and Medicare Part A clinical reviews
- Document in PointClickCare or facility EMR in accordance with CMS PDPM standards
Qualifications
- Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatrics
- Clinical comfort managing CHF, COPD, diabetes, dementia, palliative needs, and complex polypharmacy
- Prior experience or strong interest in long-term care, sub-acute rehabilitation, or skilled nursing preferred
- Active Illinois medical license (or eligibility to obtain) and DEA registration where applicable
