A Post-Acute Care Internal Medicine Physician opportunity is available in Wilmington, Delaware, embedded within a skilled nursing facility (SNF) setting operated by one of the nation's largest physician services organizations. Practice Highlights
- Provide internal medicine attending coverage to a SNF resident panel of approximately 75–125 patients
- Manage admission history and physicals, monthly recertifications, and significant-change visits
- Diagnose and treat medically complex post-acute conditions including CHF exacerbations, COPD, sepsis recovery, and post-operative management
- Lead the interdisciplinary team (IDT) alongside NPs, PAs, rehab therapy, nursing, and social work
- Document in PointClickCare or facility EMR in accordance with CMS PDPM standards
Qualifications
- Board certified or board eligible in Internal Medicine
- Proficiency in chronic disease management, anticoagulation, and palliative medicine
- Prior SNF, sub-acute, or hospitalist experience strongly preferred
- Active Delaware state license (or eligibility to obtain) and DEA registration where applicable
