A leading national physician services organization is recruiting a full-time Post-Acute Care Physician to manage a skilled nursing facility (SNF) resident panel in the Katy/Houston, TX area. Practice Highlights

  • Attending physician services for a SNF resident panel of typically 75–125 patients
  • Full-time schedule of 4–5 clinical days per week; on-call coverage included
  • Admission H&Ps completed within 48–72 hours; monthly recertifications and significant-change-of-condition visits
  • Clinical scope encompasses chronic disease management (CHF, COPD, diabetes), post-acute recovery, wound care, dementia care, complex polypharmacy, and end-of-life/palliative care
  • Collaborative interdisciplinary team model: NPs/PAs, nursing, PT/OT/SLP therapy, social work, and consultant pharmacy
  • Active participation in interdisciplinary care planning meetings, family conferences, and Medicare Part A clinical reviews
  • Documentation in PointClickCare or facility EMR, fully aligned with CMS Patient-Driven Payment Model (PDPM) standards

Qualifications

  • Board certification or board eligibility in Internal Medicine, Family Medicine, or Geriatrics required
  • Completed residency in Internal Medicine, Family Medicine, or Geriatrics; Geriatrics fellowship a plus
  • Active Texas medical license or eligibility to obtain; active DEA registration where applicable
  • Clinical comfort managing CHF, COPD, diabetes, dementia, palliative needs, and complex polypharmacy in a frail or elderly population
  • Prior experience or genuine interest in long-term care, sub-acute rehabilitation, or skilled nursing strongly preferred
